The onsite Resource Center at Reliance Medical Centers connects Polk County seniors to valuable information and essential programs through the Florida Department of Children and Families Automated Community Connection to Economic Self Sufficiency (ACCESS).
Programs available include (but are not limited to):
- Medicaid Applications
- Supplemental Nutrition Assistance Program (formerly known as Food Stamps)
- Electronic Benefits Transfer (EBT)
- Housing Assistance
- Pharmacy Assistance Programs
- OTC Benefits
A Resource Advocate will help by providing:
- Paper applications
- Access to a telephone to call DCF Customer Service
- Computer access to fill out online applications
- Printer for ACCESS documents
- Fax Machines
- Copy Machines
- Assistance with completing online applications

The Economic Self-Sufficiency Program helps to promote strong and economically self-sufficient communities by determining eligibility for food, cash, housing and medical assistance for individuals and families on the road to economic recovery.
While we cannot guarantee acceptance or approval, our Resource Advocates are happy to help connect patients to the best programs and assist with applications and paperwork. Our Resource Centers are state certified Access Centers and are equipped with computers so that you can research independently or together with our staff. This resource is available to patients and non-patient community members so that we can help anyone in need of a senior advocate.
