We are a Primary Care Provider for Adults with Medicare, focused on the needs of those living in Polk County with Medical Centers in Lakeland and Winter Haven.
While we are the place that you go for all health-related concerns and routine check-ups, we are also a Wellness Club that promotes social interaction and long-term independence. We are the collaborative partner and designated site in Polk County for the groundbreaking and much publicized USF Brain Health Program. We are the place with an on-site pharmacy, complimentary shuttle service, 24-hour Medical Priority Line, helpful Resource Center and a friendly 8-person team-approach to healthcare with our Care Focus Team. Welcome to your Care Focus Team, a coordinated group of 8 clinical professionals led by your Primary Care Physician (PCP) and Personal Medical Concierge (PMC) all working together toget you on the road to an idealized healthcare experience. This model is designed around the patient to ensure we are organized and in communication about appointments, referrals, prescriptions and all other aspects of your health. Also on the team is a nurse practitioner, referrals specialist, quality analyst, medical records specialist and two medical assistants. We look forward to welcoming you to the team approach of healthcare.
