We accept the following Value Based Medicare Advantage plans Devoted Health Care, Simply Healthcare, Freedom Health, Optimum HealthCare, Humana, and CarePlus Health Plans. As long as they are in network with your insurance. The network of specialists available to you is based on your health plan’s specialist directory, not by Reliance. If your specialist is included in your plan’s directory, we can refer you to them.
However, we encourage our patients to see our preferred specialists. We have developed a comprehensive coordination care process with our preferred specialists and they are located within our community.
On September 28, 2022, The CDC released their latest guidelines regarding masking within the healthcare setting. Subsequently, masking mandates in areas of low and medium community transmission levels have been recommended as OPTIONAL.
Following these new CDC guidelines, Reliance Medical Centers believes this as a safe practice for all – patients, staff, healthcare partners and vendors – in being given the option of masking on-site as a personal choice.
Reliance will continue to provide masks at our entrances for anyone who wishes to wear one.
Masks will be highly encouraged for those who may:
• Have suspected or confirmed COVID-19 or other respiratory infection (e.g., those with runny nose, cough, sneeze)
• Had close contact (patients and family members) with someone with COVID-19, for 10 days after their exposure
• Reside or work at a facility experiencing a COIVD-19 outbreak
At this time we are referring out for all imaging services including x-rays, MRI’s CT Scans, and Mammography services. We are happy to work with an imaging provider that is in network with your insurance if needed.
The Wellness Club at Reliance Medical Centers is no cost and available exclusively to Reliance patients on Value-Based Medicare Plans. Options include in-center Fitness, Activities and Prescribed Medical Massage.
