Welcome to your Care Focus Team, a coordinated group of 8 clinical professionals led by your Physician and APRN, and closely supported by your Personal Medical Concierge, working together to get you on the road to an idealized healthcare experience.
This model is designed around the patient to ensure we are organized and in communication about appointments, referrals, prescriptions and all other aspects of your health.
